Can I publish a post with this?

No. Every endpoint here is read-only. Publishing to Instagram goes through Meta's Content Publishing API, which needs a Business or Creator account, app review and an access token for an account you control. If posting is what you came for, that is the correct tool and this is not it.

How far back can I paginate?

As far as the account's public feed goes. Pass the next_cursor from each response into the following call.

Do the like counts update in real time?

No. A response is a snapshot from that request. Use requested_at to record when the lookup was made.

Are reels included?

Yes, reels appear in the feed with type reel. If you only want reels with play counts, use the Reels API instead.
